Office Policy Regarding Copayments and Balances

It is the policy of this office to pay the copayment at the time of service. If the patient has a balance this must be paid before rendering service or the visit will be rescheduled. Services will not be done unless the full balance is paid out. Failure to show up to an appointment with no call to cancel ahead of time will result in a fee of $50.
